ASHA CE
Transcript |
- We are no longer using
session evaluation forms for each session attended.
These are the forms that you previously completed and turned
in at the end of each session you attended, in order to get
that attendance placed on your ASHA CE transcript.
- We now have a single "ASHA
CE Participation Form" for ou to use to track all sessions
attended; this form will be available on the CE
table.
- You will still need ot
fill out ONE ASHA scantron (bubble) form in order for your
convention attendance to be listed on your ASHA CE
transcript. This form will also be available on the CE
table.
- Please remember to turn in
BOTH completed forms at the registration table BEFORE you
leave convention. If you neglect to do so, hour
convention attendance will not be printed on your ASHA CE
transcript.
